Improving life after stroke with tailored support: Innovation in use of national registry data (A-LISTS)

The purpose of this study is to co-design with clinicians, stroke consumers and health administrators a hospital-led, follow-up service for people living with stroke who report poor outcomes as part of their follow-up survey for the Australian Stroke Clinical Registry.

We are currently undertaking a randomised controlled pilot trial to test whether the program we have designed to review and manage people living in the community with stroke that we identify as vulnerable based on the registry follow-up data is feasible. This project has been funded by a MRFF 2020 Cardiovascular Health grant.

Aim
